I do hereby declare and acknowledge that the information herein stated, is, to the best of my knowledge, true and correct.我谨此声明并确认，以下所陈述的资料是真实正确的。
I understand that I must submit this form under my individual name, and that I am 18 years old or above.我明白此表格必须以本人名义单独提交，且本人已年满18岁。
I further declare that I will observe and abide by the rules and regulations stipulated in the Dhamma Vinaya and also that of Santi Forest Monastery.我将遵守并奉行法的戒律和寺院的条规。
I understand that Santi Forest Monastery will not be held responsible for any physical or mental injury arising during and/or after attending this programme.寺院将不对任何一位的身体或精神伤害负责。
I accept that Santi Forest Monastery reserves the right to request me to leave the monastery at anytime.寺院有权要求任何一位立即离开道场。
